樸次茅斯骨外固定器治療開放性脛骨骨折的臨床報告

Management of Open Tibial Fracture Using the Portsmouth External Skeletal Fixator

摘要

長庚紀念醫院骨科，從1979年6月至1982年3月，使用樸次茅斯骨外固定器，治療開放性脛骨骨折，一共71例。病患年齡由6歲至59歲不等，平均外固定器使用時間是105天，其中36例在本院治療至骨折癒合，未接受植骨術者27例，其平均骨癒合時間是7 1/2 月，接受植骨術者9例，其平均骨癒合時間是14月。樸次茅斯骨外固定器，能提供骨折部位相當穩定的復位和固定，讓受傷的患者早期立床活動，並能方便傷口照顧和分期重建手術的遂行。由本篇的臨床研究，我們認為樸次茅斯骨外固定器是個簡便易行、價格便宜，對開放性脛骨骨折極有幫助的骨外固定器。

There were 71 cases of type III and high grade type II open tibial fractures with major associated injuries treated with the Portsmouth external fixation in Chang Gung Memoiral Hospital between june 1979 and March 1982. The age was from 6 to 59 years (average 35 years). The average hospitalization time was 51 days. The average external fixation time was 105 days. Thirty-six cases were followed up until fracture union. The average union time of no bone graft group (27 cases) was 7½ months. The average union time of bone graft group (9 cases)was 14 months. The Portsmouth external fixator provided stable fracture fixation and early mobilization which facilitated critical care of polytrauma victims, local wound management and staged soft tissue reconstruction. From this study, we concluded that the Portsmouth external fixator was very simple, cheap and helpful in the management of high grade open tibial fractures.
