自民國73年7月至74年12月,榮民總醫院一共收療了105位含107例開放性脛骨骨折。患者依照骨折部位的軟組織損傷程度,參考GUSTILO分類法分類,追蹤時間14個月。計有第I型58例,第II型29例,第III型20例。一搬的處理方式是患者一到達急診處,便先接受預防性的抗生素靜脈注射,以及初步的患部清洗和暫時性的外固定術。若狀況穩定,則儘速送往手術室,接受進一步的擴創術以及大量的沖洗,接著施行骨折整復手術及內或外固定術。第I型37例使用鋼板內固定術,17例接受骨髓內釘固定,4例使用石膏固定。第II型有15例使用鋼板內固定,6例接受骨髓内釘,6例接受外固定器固定2例則用石膏外固定。第III型仍有5例使用鋼皮內固定,1例骨髓內釘,13例使用外固定器固定。手術後,第I型則給予3天抗生素靜脈注射,第II型3至5天,第III型則至少7天。發生術後的傷口感染率為第I型8.6%,第II型20.7%,第III型40%。平均18%。偏高的總感染率可能是由受傷至進行治療中間的時間延誤。但值得提出來的是8小時內接受手術治療和16小時內接受治療者相比較,其感染率卻未見上升。此一數據或許顯示,若患者在超過8小時以上的延誤,而仍在16個小時的範圍內,可以視狀況接受內固定手術之治療。也就是說,所謂「黃金時段」,或可延長至16小時,此一觀點當然需要更多的事實支持。

During the period from 1984 to 1985, 105 patients with 107 open fractures of tibia were treated at Veterans General Hospital in Taipei, Taiwan, Republic of China. Data were analyzed retrospectively. Fracture types were classified according to the severity of soft tissue injury, because the initial injury was the most important prognostic factor. Two patients received amputation, 7 tibias received casting immobilization, 18 cases received external skeletal fization, 24 cases received medullary nailing and 57 tibias received open reduction with plate fixation. The overall infection rate was 18%, nonunion was noted in 11 patients. By the retrospective study of the improved surgical technique, new trend antibiotics protocol, and the results of our seried, the concept of Golden period maybe extended up to 16 hours without the risk of increasing infection rate.
