Compartment Syndrome of Lower Leg in a Late Arrival Snakebite Patient – A Case Report

蛇咬病患下肢隔腔症候群-病例報告

摘要


亞熱帶地區常見蛇咬病患,然而,因蛇咬導致隔腔症候群(Compartment syndrome)的併發症卻屬罕見。此狀況常導因於出血性蛇毒咬傷,且通常咬傷深及肌肉層,抗毒血清劑量不夠或延遲給予。因為臨床上罕見,故常被忽略而引致嚴重後遺症。 本文報告一病例,經適當治療後功能恢復良好。並參考文獻,探討適當防治方法。臨床上須提高警覺,特別注意蛇咬病患的肢體循環,必要時須測量隔腔壓力,並及早施行肌膜切開術,以防範續發性肌肉壞死的發生。

Snakebite is a common injury in subtropics. However, compartment syndrome due to snakebite is a rare complication. In this paper, a 52 year-old man suffering snakebite complicated with compartment syndrome of the loser leg was reported. Twenty hours passed over after snakebite until to fasciotomy. The compartment syndrome of lower legafter snakebite usually results from deep inadequate bite, inadequate dose of antivenins, and delayed treatment. Because of its rare incidence, it is frequently neglected and disastrous sequelae may come from delayed management. Although the venom itself may result in irreversible necrosis of the muscle, however, early fasciotomy may prevent further secondary necrosis due to increased intracompartmental pressure. Routine check of the clinical condition and measurement of the intracompartmental pressure are essential to prevent disastrous sequelae of compartment syndrome in a snakebite patient.
