摘要

股骨遠端骨折是一種較為複雜且嚴重的骨折，常發生在車禍和年者的病患，多數以手術治療為主。目前可使用的內固定器很多，但均有操儘不易的缺點。
本院自民國73年2月起，開起運用Judet鋼板系統在股骨遠端的骨折上；由於它鋼板幅度相似於股骨遠端的外側，加上容易操作及堅固，所以延用至今。在這六年中，一共運用在69位病患身上；在可追蹤的的55位患者，年齡從16至76歲，平均為50歲，追縱時間從3個月至71個月，平均為37.9個月；最後統計發現；在新的骨折患者中，有78%患者達到滿意程度，而在陳舊性骨折中，有67%患者達到滿意程度。
因此我們認為在股骨遠端骨折中，其若無法以Interlocking nail加以固定，則Judet 鋼板系統亦不失為可運用的內固定器之一。

between 1984 and 1989 the Judet plate system was used to fix 69 femurs, which were not suitable for inerlocking mail fixation. In 55 patients who coule be traced, 10 patients had supra- & intercondylar frature,27 patientshad supracondylar fracture, and 18 patients had old distal femoral fracture after other treatment with non-union. A high rate of union (94.5%) was achieved in this group, which included a variety of fracture types. Knee motion exceeded 90°in 85% of supracondylar fracture group, 60% of supra- & intercondylar fracture group had 67% of old fracture group.
Excellent or good results were achieved in 74.6% of patients. Fair results were noted in 16.4% of patients and results in 9% were considered failure. The curvature of judet plate is similar to the antero-lateral slope of the distal femur, and the apparatus has the advantages of easy application, rigid fixation and minimal failure rate.
