Percutaneous Transverse Pinning for A Rare Complicated Fracture Injury of the Second through Fifth Metacarpals - A Case Report

Background: Closed metacarpal fractures involving the first to fifth metacarpals are very rare. Multiple metacarpal fractures are usually associated with significant soft tissue swelling. Early stable fixation and functional rehabilitation optimizes outcomes. Herein we present an easy way to treat such a complicated fracture to preserve the hand's aesthetic appearance and functionality. Materials and Methods: An 18-year-old man who was involved in a motorcycle accident experienced contusions to his left hand. Plain radiography indicated multiple closed metacarpal fractures involving the first to fifth metacarpals. We used percutaneous transverse pinning method for treatment in this case. Results: Plain radiograph showed fair union and alignment of the metacarpal bones. Good aesthetic and functional results have been observed after a 5-month follow-up. Conclusion: Percutaneous transverse pinning is an easy way to treat multiple closed contiguous metacarpal fractures.
