Management of Complicated Distal Third Femoral Fractures Using the Retrograde Intramedullary Supracondylar Nail

From September 2001 until August 2003, a retrospective study of nine patients with ten displaced distal femoral supracondylar fractures, who were treated by open reduction and internal fixation with the retrograde intramedullary supracondylar nail (IMSC), was carried out. According to the AO/OTA classification, the fractures consisted of three cases of type A1, and six cases of type A3 fractures. All patients were followed up for over one year. The clinical results were reviewed. Five of nine fractures healed clinically and radiographically within 6 months after operation. Four fractures resulted in non-unions with broken distal locking screws or screws loosening. Re-operation with autogenic bone grafting was performed. There was no breakage of the nail. Physically and mechanically, the IMSC nail was strong enough to secure the fractures, and lead to heal. The retrograde IMSC nail for distal femur fracture fixation can be an alternative choice in some circumstances.
