Ender's Nailing in the Treatment of Tibial Shaft Fractures

自1986年五月起到1989年十月止，共有41個病人因脛骨骨折在台北市中興醫院接受Ender氏釘治療。住院時男性病人的平均年齡為35.2歲（從17歲起到78歲止）;女性病人的平均年齡為33.6歲（從18歲起到64歲止）。平均追蹤期間為16.1個月（從5個月到46個月）。依據Hamza氏所訂下的標準，骨折治療的結果有8個病人被評定為極優;有21個病人被評定為優;有11個病人被評定為尚可。有1個病人失去追蹤。極優和優的病人所佔的比例為72.5%。併剸症包括壹個病人傷口深部感染;有兩個因釘子刺激引起不快感;有四個足踝僵硬;其中包括兩個釘子突出的病例。不過，所有的病人骨折均癒合，儘有壹個病人癒合時間為九個月。有五個病人癒合的角度不佳，其中有四個為脛骨下端一骨折。依據本篇治療骨折的經驗，Ender氏釘不失為治療脛骨骨折既簡單，便宜又有效的工具。最適合用在中段的骨折。勉強可用在上段或下段的脛骨骨折。

From May 1986 to October 1989, 41 patients (41 fractures) with tibial shaft fractures were treated with Ender nails at Taipei municipal Chuang-hsing Hospital. There were 35 males and 6 females. The mean age of the male patients were 35.2 years-old. (range, 17 to 78 years-old) and that of the females were 33.6 years-old (range, 18 to 64 years-old ). The follow up period was 16.1 months in average (range, 5 to 46 months). According to the criteria of K. N. Hamza, the results were graded excellent in 8 patients, good in 21 patients, fair in 11 patients and no patient was graded poor. The satisfactory rate (excellent and good) was 72.5%. The complications included 1 case with deep wound infection, 2 with nails irritation, 5 with significant malalignment, 4 with stiffness of the ankle including 2 cases of nails protrusion. However, all the fractures treated in our hospital by Ender nails showed union in spite of one delayed union (9 months). Of the 5 malunion cases the function did not impair significantly and 4 cases belong to the group of lower third tibial fracture. According to our limited experience and cases, we recommend Ender nails as simple, effective and cheap instruments for the treatment of middle third tibial fractures. It can be used on segmented, proximal or lower third tibial fractures under the disadvantage of more complications such as malunion.
