不理想斷肢再接術之檢討

摘要


斷肢再接術之目的並非只是求肢體的存活,更為重要的是求機能的恢復,而外表的美觀,則只是一種額外的收獲。我們長期追蹤40個斷肢重接之功能,雖然存活率可達87.5%,但臨床上機能恢復達滿意程度僅達70%。本文探討成績不佳之原因歸納為病人選擇不對,技術失敗,患肢感染,以及肌腱黏連等,此種分析可助於我們如何愼選病人,技術上改進以及併發症之處理以提升手術成績。

A successful replantation of mutilated extremities should have not only the certainty of survival but also the maximum functional recovery. In this study of 40 replantation units after more than 5 years’ follow-up only 70% achieved good or excellent result. Five cases failed in replantation due to wrong indication orthrombosis of vessels. The causes of unsatisfactory replantation were two level amputation, technical failure, infection and adhesion of tendons. Careful selection of patients, improved surgical skill, and good handling of tendons will surel improve the result of replantation in the future.
