以掃描式電鏡研究灌流液對軟骨微結構之影響

Irrigation Fluids for Arthroscopy: A Scanning Electron Microscope Study

摘要


我們評估了四種關節鏡檢查或手術時使用的灌流沖洗液對關節軟骨表現顯微結構是否有影響我們用平常關節鏡檢查使用的生理食鹽水及乳酸林格氏液,及電刀手術時使用的非導電性去離子純水及山梨醇0.3%溶液,來灌流沖洗小白鼠(Spraque-Dawley)的膝關節,再利用掃描式電鏡來觀察髕骨表面軟骨之顯微結構。我們並沒有將軟骨切片取下觀察,而是將整個髕骨及其表面軟骨一同固定,鍍金,然後以掃描式電鏡來觀察。藉以避免因切片過程造成人為瑕疵,影響判讀。結果發現這四種溶液及沒有灌流的對照組,彼此之間並沒有任何明顯差異。我們認為生理食鹽水,乳酸林格氏液,去離子純水,及山梨醇0.3%溶液對關節軟骨皆無負面影響,可安全使用於關節檢查及手術。

Four irrigation solutions were evaluated for their effects on the ultrastructure of the articular cartilage surface utilizing scanning electron microscope(SEM). The knee joints of rats were randomly unirrigated as control, or irrigated with normal aline, Ringer’s lactate, 3% sorbitol,or distilled water, respectively. The articular cartilage of the patella was kept undisturbed from the underlining bone. Results of the study revealed no significant differences among the four solutions and the unirrigated control group. It is concluded that Ringer’s lactate, normal saline, distilled water, and3% sorbitol can all be safely used as irrigation solutions in arthroscopic surgery.
