The Modified Bristow Procedure for Recurrent Anterior Shoulder Dislocation

摘要


1984年一月至1990年十二月,計有52例重覆性前肩關節脫臼病患,接受Modified Bristow Procedure。其中32例接受追蹤檢查,平均追蹤時間為26個月。38例中只有一例因鏍絲固定位置偏高而再脫臼,餘都再無脫臼發生。術後理學檢查,肩關節外轉平均喪失15度,內轉喪失8度,其他方向活動幅度良好。X rays術後檢查顯示喙突移殖骨塊癒合良好佔82%,被吸收及脫臼佔18%,肌力檢查與正常側比較並無減弱現象。74%病人術後運動正常活動無限制。18%病患則有稍微投擲運動受限,8%上臂上舉過肩有運動受限,病患滿意度為82%。以Modified Bristow Procedure治療重覆性前肩關節脫臼非常有效,唯術後肩關節外轉受限造成投擲運動的不便是手術前必須考慮的問題。

A follow-up study of 38 out of 52 patients operated on for recurrent anterior shoulder joint dislocation according to the modified Bristow procedure during the year 1984 through 1990 in Tri-Service General Hospital. The average follow-up period was 26 months. Postoperatively, there was one of recurrence due to highly position of the fixation of the screw. The average loss of external rotation as compared to the non-operated side was 8° in adduction and 15° in abduction. Seventy-five percent of the patients felt that there was no limitation of their activities postoperatively. Eighteen percent felt limiation in throwing sports and eight percent felt limited in swimming and tennis. Shoulder pain was found in 20% of the patients during forcefully external rotation of the shoulder. Radiographic studies revealed the union rate between the bone transplant and the glenoid rim was eighty percent. Patients wre satisfied with 82 point as compared to the preinjury status.
