Although carpal tunnel syndrome is a common compression neuropathy of the upper limb, a review of the literature shows that compression of the median nerve by tophaceous deposit involving the flexor tendons is not common. We experienced and successfully treated a similar case in which the flexor pollicis longus tendon was in-volved. Although medical therapy has been claimed to be effective in the treatment of carpal tunnel syndrome secondary to gout, we believe that surgical decompression is the treatment of choice as it helps confirm the diagnois and give immediate symptom relief. Antigout therapy should also be given to ensure proper wound healing and to diminish the chance of recurrence.
