Instability of Distal Radioulnar Joint after Operative Treatment of Galeazzi's Fracture: A Report of Two Cases

橈骨遠端骨折(Galeazzi Fracture)手術內固定後引發遲發性遠端橈尺骨關節不穩定—病例報告

並列摘要

Distal radioulnar joint instability following the Galeazzi fractures is rare. We present two patients who suffered from late instability of the distal radioul-nar joint following galeazzi fracture. Both patients had anatomical reduction of the wrist and forearm after receiving open reduction and internal fixation. Long-arm casts were applied n supination of the forearm for six weeks. Unfortunately, both patients suffered from painful instability of the wrists one to two weeks after removing the casts. Physical examination and plain radio-graphs revealed subluxaton of the distal radioulnar joint. The male patient received wrist arthroscopic examination. A tear of the triangular fibrocartilage complex was found and open repair of the triangular fibrocartilage complex was performed. Range of motion of the left wrist of this patient was regained with-out pain. In contrast, the female patient refused further management and had persistent painful limitation of range of motion of the left wrist and weakness of grip strength. Although Long-arm castig in the position of forearm supination for six weeks after open reduction for the radius is a standard management for Galeazzi fractures, it does not guarantee a normal wrist for patients suffering from severe derangement of distal radioulnar joint with inadequate healing of triangular fibrocartilage complex.
