Vascular complication in arthroscopic-guide reduction and internal fixation (ARIF) of the tibial fracture is very rare. We reported a case of 40-year-old man who sustained left tibial plateau fracture and was treated by ARIF with cannulated screws. Six weeks later, a pseudoaneurysm from left tibioperoneal trunk was proven by MRI, CT and angiography. In conclusion, MRI provided useful information for preoperative planning.
