台北社區居民凝血檢驗指標之常模評估

由於疾病和健康是一種相對的觀念，況且絕對健康實際上也不存在，因此爲了解釋實驗室數據，所有檢查均需建立生物參考區間，但目前因凝血試驗很少被系統化或常規化的呈現，所以本研究目標爲觀察30歲以上且無心血管疾病之社區居民中，凝血試驗數據在性別及年齡是否有差異，並以95%信賴區間作爲正常族群之參考區間。研究分析項目包括有凝血酶原時間(Prothrombin time, PT)、國際正規化比率(International normalized ratio, INR)、纖維蛋白原(Fibrinogen, FIB)、部分凝血酶原時間(Activated Partial Thromboplastin Time, APTT)、第三抗凝血酶(Antithrombin Ⅲ, AT-Ⅲ)與蛋白C(Protein C, PC)。結果本研究依年齡和性別分別列出PT、INR、FIB、AT-Ⅲ與PC的生物參考區間，並觀察到FIB、AT-Ⅲ、PC主要有性別上的差異；此外，APTT、AT-Ⅲ有年齡上的差異。但FIB雖無觀察到年齡顯著差異，卻發現到有隨著年齡增加而增加的趨勢。

Normal is relatively to disease in general, but absolute normal is not existed. Additionally, coagulation test has an increasing importance recently. However, normal criterion in laboratory usually used the existed information commercially, and the systematic reference range still has not provided. We aimed to present the distribution of Prothrombin time (PT), International normalized ratio (INR), Activated Partial Thromboplastin Time (APTT), fibrinogen (FIB), Antithrombin Ⅲ (AT-Ⅲ), Protein C (PC) in healthy-like residents who aged over 30 year-old and without heart disease from Shih Lin community in Taipei. We also aimed to explore the differences in 3 age groups (30-45 yrs, 45-60 yrs and 60+yrs) and in both genders. We reported the reference ranges of PT, INR, APTT, FIB, AT-Ⅲ, and PC by age and gender. The reference ranges of above clotting tests were showed according 95% confidence interval in this paper. We also found the differences in various age groups in APTT and AT-Ⅲ, and in gender in FIB, AT-Ⅲ and PC. An interaction of age and gender was reported in PC.
