Functional Knee Performance in Patients Following Unilateral Femoral Shaft Fractures

單側大腿骨幹骨折病人之膝關節功能表現

摘要


本實驗研究十一位單側大腿骨幹骨折後五年以上病人膝關節的功能表現。測試分兩部份:骨折側與非骨折側的等速肌力值以及膝關節功能表現測試。等速肌力值包括在兩種不同角速度下(60deg/sec及180deg/sec),股四頭肌及大腿後肌的最大肌力以最大等速收縮力矩為代表,大腿後肌之於股四頭肌的肌力比值與相同肌群左右兩腳的相對肌力比值。膝關節功能表現測試包括步態、上下樓梯、深蹲(次數與對稱性)以及單腿前躍等四項以作為日常生活處理能力的代表。同時將上述兩組資料做相關係數和下肢對稱性分析。結果顯示,骨折側的股四頭肌有明顯的肌力降低及較差的單腿前躍功能表現。單腳前躍與同側的股四頭肌與大腿後肌肌力有明顯的正相關。骨折側的大腿後肌肌力與對側和比則沒有顯著差異。其他的四項膝關節功能測試受試者均有正常或接近正常的表現,同時也與等速資料沒有顯著的相關性。因此在臨床上,等速肌力的測試結果不應單獨做為評估的標準而須參考其他資料以決定病人情況是否足以應付日常生活事務及休閒活動之所需。

This study investigated the relationship between functional knee performance and isokinetic data in 11 patients at an average of 81 months post unilateral femoral shaft fracture. Functional knee performance test, including walking, stair climbing, squatting, and one-leg hopping, and isokinetic tests of the hamstrings and quadricpes muscles were performed. The peak torques, hamstrings to quadriceps ratio, and relative strength deficits were collected during the isokinetic test at 60 and 180 deg/sec. Results showed that the mean strength of quadriceps on the fractured limb was significantly lower than that on the non- fractured limb. No difference in peak torque of the hamstrings was found between legs. The results of the one-leg hop test on both legs had significant correlations with peak torques of the hamstrings and the quadriceps on respective leg. No significant correlation between the isokinetic data and other three functional performance tests was found.
