Successful Allogeneic Hematopoietic Stem Cell Transplant in a Patient with Acute Lymphoblastic Leukemia Following Total Marrow Irradiation

摘要

全骨髓照射及全骨髓／淋巴照射是近年來發展的新型放射治療，用於血液惡性疾病接受幹細胞移植之病患可能有潛在好處。我們提出一位急性淋巴性白血病病人，在異體血液幹細胞移植前，接受全骨髓照射合併高劑量etoposide作為調理療法（conditioning regimen）。此療法只造成病人低毒性並且成功植入幹細胞。由於最佳的放射劑量，所造成的併發症和治療成效目前仍不明確，值得進一步的前瞻性研究來證實。

Both total marrow irradiation and total marrow/lymphoid irradiation have been developed recently and may achieve potential advantages in patients with hematological malignancy undergoing stem cell transplantation. We present one patient with acute lymphoblastic leukemia treated by total marrow irradiation and high-dose etoposide as conditioning regimen before allogeneic hematological stem cell transplant. Lower toxicities and successful engraftment were noted. The optimal radiation dose, complications and treatment efficacy of total marrow irradiation are not clear and deserve further prospective studies.
