A 21-year-old female presents fever, jaundice, pancytopenia, hepatosplenomegaly running a fulminant course simulating malignant histiocytosis. Necropsy proves peripheral T cell lymphoma with prominent hemophagocytosis in liver, spleen and bone marrow. In situ hybridization confirms EBV genomes in the nucleus of tumor cells. The pathogenesis is discussed.
