出院後結核病人公衛長期追蹤之價值

摘要

民國74年末，花蓮門諾醫院小兒科醫師有鑑於科內小兒結核病人數驟增，便極力探求此一現象背後真正的原因。經過公衛部門積極查證，肯定全院結核病患在該院該年突有直線上升的現象，尤其是在幼年和老年的人口群中。經過多次的反應與輾轉爭取，終於取得中央防治單位的同意，在花蓮縣展開一串大規模的結核病篩檢。並經由篩檢結果確定有個結核病小流行在花蓮縣秀林鄉形成，令人驚訝的是該院在時間與地點的預測旣與篩檢結果不謀而合。這結果不僅肯定了基層醫療人員對於警訊所作的積極反應，更充分說明了地區與中央協同合作的公衛網路的重要性。

It has been varied in opinions in our country about investigation in public health affairs. How far should one invest? All has been deeply concerned. In our study, the tuberculosis patients had been constantly followed up by public health department. With this simple monitoring system on patients number, age and distribution, and the cooperation of staffs in the hospital, we accidentally detected one tuberculosis endemic event in Shiu-Lin rural village, Hualien, Taiwan. This exciting result give us a positive answer to those concern, which we have mentioned before.
