Peptic Ulcer with Double Pylorus – Report of Two Cases

摘要

雙幽門是一種較少見的情形，是由胃之前庭與十二指腸之間因廔管形成溝通所組成。本文報告近五年來在本院經由上消化道內視鏡檢查診斷為消化性潰瘍併雙幽門之兩病例。病例一是位六十三歲男性病人，於民國七十七年經內視鏡檢查診斷為消化性潰瘍。其後數年患者有反覆上腹痛之症狀。一系列之內視鏡檢查發現於民國八十一年及八十二年時有雙幽門形成。病例二是位七十二歲女性病人，於民國八十一年經由內視鏡檢查診斷為胃潰瘍。四個月後之追蹤內視鏡檢查顯示胃潰瘍併雙幽門。患者於接受治療後三個月之第三次內視鏡檢查顯示胃潰瘍瘢痕並無雙幽門之存在。本文報告兩病例之臨床過程以及有關雙幽門文獻之回顧。

The double pylorus is a rare condition consisting of fistulous communication between the gastric antrum and the duodenum. We report here two cases of peptic ulcer with double pylorus which was diagnosed by means of upper gastrointestinal endoscopy at our hospital in recent 5 years. Case 1 is a 63-year-old male patient who was diagnosed to have peptic ulcer by endoscopic examination in 1988. Recurrent epigastric pain occurred in the following years. Serial panendoscopy in the following years discovered double pylorus formation in 1992 and 1993. Case 2 is a 72-year-old female patient who was diagnosed to have gastric ulcer by panendoscopic examination in 1992. Follow up panendoscopy 4 months later showed gastric ulcer with double pylorus. The patient received medical treatment and the third panendoscopy 3 months later revealed gastric ulcer scar without the presence of double pylorus. The clinical courses of the two cases were presented and the literatures concerning double pylorus were reviewed.
