Renal Transitional Cell Carcinoma with Tumor Thrombus intot the Vena Cava：A Case Report

摘要

一名68歲女性因偶發血尿以及近來間斷性腰痛而求診。超音波及電腦斷層檢查顯示在右腎有一巨大腫瘤同時伴有下腔靜脈局部侵犯，尿液細胞學檢查並未有異常發現。初步診斷為腎細胞癌，隨後據此施行根除性腎切除併下腔靜脈栓塞截除術。術後病理診斷為移形上皮癌合併下腔靜脈侵犯。腎細胞癌偶而會伴有下腔靜脈侵犯，但移形上皮癌則極其罕見。目前為止英文文獻上僅有20例報告。病人在術後11個月的門診追蹤並無復發之跡象，然統計上指出此類病人預後相當不好。我們特將此案例提出報告，同時作一文獻上之回顧。

A 68-year-old female patient had an episode of hematuria in June 1999. She also complained of intermittent right-sided flank soreness. Computed tomography revealed a 3.7×4.8 cm. Mass occupying the upper portion of the right kidney with inferior vena cava (IVC) thrombus. Under the suspicion of renal cell carcinoma (RCC), radical nephrectomy with IVC thrombectomy was carried out subsequently. Pathologic examination of the specimen confirmed transitional cell carcinoma (TCC) without regional lymph node metastasis. There was no evidence of recurrence postoperatively during a 11-month period of follow-up. Invasion of the inferior vena cava by RCC is rare, but there are a few reports of such local extension of TCC. Because most patients have advanced disease at the time of diagnosis, the prognosis is grim. Herein we report the case and literature review.
