AFTERLOADING TECHNIQUE IN THE RADIOTHERAPY OF CARCINOMA OF CERVIX AND BODY OF UTERUS

From the beginning of the twentieth century, intracavitary radium application has become the primary treatment for carcinoma of the uterine cervix. A survey conducted in the United States revealed that less than 10% of patients with cervical cancer were treated by surgery alone, in the 1940s as well as the 1960s. With elaborate combinations of external pelvic irradiation, almost all stages of infiltrative carcinoma of the uterine cervix can be treated by radiotherapy alone. The development of the Fletcher-Suit afterloading technique offers the best method of intracavitary treatment; no radiation exposure occurs in the operation room or the recovery room, the applicator is flexible to meet various clinical situations, the vaginal mucosa and the urinary bladder have lesser chance being over treated. The afterloading technique can be applied to certain cases of carcinoma of the body of the uterus, either as preoperative radiotherapy or radiotherapy alone. However, the successful application of afterloading technique needs thorough understanding of radium physics, the nature of the disease, and the principles and techniques of treatment.
