摘要
一個一歲半的幼兒，經急診室住進本院小兒科病房。主訴高燒和咳嗽己有一週之久，住院之前孩童隨父母去看私人診所，醫生認為上呼吸道發炎，雖經治療但不見效，因高燒仍存在送到本院急診室觀察，在急診室時立卽接受胸部X光照像，發現左肺門附近有浸潤現象，心臟陰影移向右肺，疑似肺炎，同時亦注意到左側橫膈膜整個升高，無肋膜積水現象，因此小孩在佳院期間除給予適當抗生素治療肺炎外，經過許多X-射線與核子醫學的檢查來探討在橫膈膜上升之原因，最後影像判讀是橫膈膜腹臟膨出。

A one and half year old male child was admitted on account of fever and cough for about one week without response to medical treatment on private clinic. Chest X-ray examination revealed infiltration in the left hilar region, pneumonia was suspected. In addition, elevation of left hemidiaphragm was likewise noticed. Further examinations such as upper G-I series, fluoroscopic observation of diaphragmatic movement and pneumoperitoneal radiography were performed to investigate the cause of elevation of the left hemidiaphragm. Liver scanning suggested an enlarged left lobe of liver beneath the hemidiaphragm but could not rule out accessory spleen. The final diagnosis was made with ^(131)I rose bengal study as eventration of the left hemidiaphragm associated with an accessory normal liver tissue in connection with left hepatic lobe. No any further treatment was given for this asymptomatic patient. Literature was reviewed to evaluate the incidence of eventration of diaphragm in children, different definitions and mechanisms of eventration of diaphragm were discussed.
