摘要


橫膈膜疲乏是橫膈吸肌於持續性或反複性收縮時,無法維持預期的收縮力。此為導致呼吸衰竭的原因之一。本篇綜合有關之研究報告,探討造成疲乏的原因、診斷及治療。橫膈膜雖是骨骼肌,但其特性介於四肢骨骼肌和心肌之間,所以可做規律的收縮,而不需長時間的休息。然而能量的供給不敷需求等因素常會造成橫膈膜疲乏。近年來,專家提出特殊的訓練方法,來加強橫膈膜的肌力和耐力,避免疲乏的產生。其肌力和耐力的訓練,分別以「吸氣阻力增加」和「再呼吸」訓練最為有效。

Diaphragmatic fatigue is a consequence of failure to maintain expected force with continue or repeated contraction of the diaphragm. It is now suggested that diaphragmatic fatigue may result in respiratory failure. This article presents the literature review related to the cause, assessment and management of diaphragmatic fatigue. According to the studies, the biological characteristics of the diaphragm is intermediate between the limb skeletal muscle and the cardiac muscle. Therefore, the diaphragm is rather resistant to fatigue and it can contract periodically without prolonged rest. The major factor of diaphragmatic fatigue may primarily due to that the energy demand exceeds the supply. Recently, special exercise programs for improving the diaphragmatic strength and endurance are reported. The inspiratory resistive loading and rebreathing methods are very effective in increasing the diaphragmatic strength and endurance, respectively.
