手術後電子射線治療以預防蟹足腫及肥厚性瘢痕之復發

POSTOPERATIVE ELECTRON BEAM IRRADIATION IN THE PREVENTION OF KELOIDS AND HYPERTROPHIC SCARS

摘要


自民國68年8月至71年11月,三軍總醫院放射治療科曾對32位患者的33處蟹足腫(keloids)及肥厚性瘢痕(hypertrophic scars)給予手術後電子射線(electron beam)放射治療,以預防其復發。32位患者中,女性21位,男性11位。年齡的範圍從10歲至68歳,平均年齡28.2歳,中値年齡24.0歲。大部分患者於手術後一星期内接受放射治療。治療的劑量爲1800-2400 cGy(rad)/6~8次/8〜11天。患者治療後至本統計至少追踪半年以上。33處病灶中17處(52%)完全治癒,8處(24%)有部分或不完全復發,另8處(24%)復發。所有患者皆無併發症產生。本統計顯示手術後電子射線治療方法簡單容易,照射部位的皮膚耐受性良好而且能有效地預防此病灶的復發。

Thirty-three clinically diagnosed keloids and hypertrophic scars in 32 patients received postoperative electron beam irradiation from August 1979 to November 1982 in the Radiation Oncology Center, Tri-Service General Hospital for the prevention of its recurrence after excision. The age ranged from 10 to 68 with an average age of 28.2 and a median age of 24.0 years old. Most patients received electron beam therapy of 1800-2400 cGy delivered in 6-8 equal doses in 8-11 days. The first treatment was delivered within several hours in 10 patients, 1-3 days in another 10 patients, and 1-3 weeks interval in the rest. With a minimum of 6 months follow-up, the cure rate was 52% (17/33), the partial response rate was 24% (8/33), and the recurrence rate was 24% (8/33). No side effects or complications during or after treatment were observed. It suggested that electron beam therapy represents a simple and well tolerated method that can achieve low recurrence rate in the prevention of keloids and hypertrophic scars after excision.
