摘要


鐮狀靭帶(falciform ligament)位於肝臟左葉的內側節(medial segment)及外側節(lateral segment)之交界處,其內有退化的臍靜脈及數條臍旁靜脈(paraumbilical vein)。當門靜脈壓增高時,臍旁靜脈變粗形成一側枝循環,上述結構以超音波掃描時為一管狀無回音之結構;以電腦斷層掃描時,在注射對比劑前似一低密度之病灶,但在注射對比劑後其密度增高非常明顯,可高達100Hu,顯示其為血管構造。本文報告一例病理證實有肝硬化之病人,起初超音波及電腦斷層檢查均懷疑肝臟左葉內側節內有病灶,後經再次的仔細超音波及電腦斷層掃描,才發現該病灶乃為變粗的臍旁靜脈。

The falciform ligament located between the medial and lateral segaments of the left hepatic lobe, recanalization of the paraumbilical vein within the falciform ligament can be found in patients with portal hypertension to serve as a collateral venous pathway. The patent paraumbilical vein is echo free tubular structure and continuous with the left portal vein, it also can be enhanced as a vascular structure in CT scan though it revealing a hypodense structure before contrast injection and simulating a hepatic tumor. The appearance of recanalized paraumbilical vein in Ultrasound, CT scan and angiogram is present here in one patient with documented liver cirrhosis.
