摘要


脊椎後縱韌帶骨化(ossification of the posterior longItudinal ligament 以下簡稱OPU)是較不常見的脊椎病變。臨床上大部份病人沒有症狀,但嚴重者可至癱瘓。在放射學檢查方面,以電腦斷層最能將骨化韌帶的形狀及範圍詳示出來。榮民總醫院四十例頸部OPLL患者中,以四肢無力或感覺遲鈍者較多。電腦斷層脊髓攝影(CT myelography)同時可知道骨化韌帶之範圍及脊髓受壓迫程度,是手術前評估的最佳方法。

Ossification of the posterior longitudinal ligament (OPLL) is an. uncommon disease involving the spine. The first case was reported by Tsukimoto in Japan in 1960. Terayarna in 1964 introduced the term "OPLL of the cervical spine" The etiology and pathogenesis of OPLL are not completely understood. We collected forty cases of cervical OPLL diagnosed by high resolution CT scan from 1000 cervical examination during the last two years. Clinical presentations include finger numbness, spastic gait, sensory impairment, motor weakness et al. Plain C-spine and tomography demonstrate OPLL in 25 of 34 cases (74%). CT myelography is valuable in identifying the shape, extent and severity of GPLL. Moreover, CT myelography is important in surgical planning of patient with OPLL.
