Adrenal Myelolipoma: Case Report and Review of the Literature

摘要

腎上腺骨髓脂肪瘤是一種含有造血組織及脂肪組織之罕見良性無內分泌功能的腫瘤。大部份之病例都在屍體解剖時偶然發現。我們報告三例腎上腺骨髓脂肪瘤及其影像學所見。其中二例在手術前即因超音波掃描及電腦斷層攝影之特殊發現而得到正確之診斷。另外一例則在手術前診斷為後腹膜腔脂肪肉瘤。後腹膜腔脂肪肉瘤常列入腎上腺骨髓脂肪瘤之鑑別診斷。對於無法確定診斷之病例，建議實施電腦斷層攝影或超音波掃描指引下之細針抽吸生體切片檢查或剖腹探查。近年來因超音波掃描及電腦斷層攝影被使用於腹部疾病檢查日增，更多之腎上腺骨髓脂肪瘤的病例將會被診斷而不是在屍體解剖偶然發現。

Adrenal myelolipoma is a rare, benign, endocrine non-functioning neoplasm which consists of hemopoietic tissue and adipose tissue. Most of the cases had been incidentally found at autopsy. We report three cases of adrenal myelolipoma in which two cases were correctly diagnosed pre-operatively by means of sonography and computed tomography. The other case was diagnosed as retroperitoenal liposarcoma pre-operatively which is usually included in the differential diagnosis. In equivocal cases, computed tomography guided fine needle biopsy or exploratory laparotomy is recommended. With increasing use of sonography and computed tomography recently, more cases of asymptomatic adrenal myelolipomas will be diagnosed in living patient rather than at autopsy.
