磁振血管攝影之臨床經驗及其原理之探討

摘要


磁振血管攝影是利用血管中的血流在磁振造影中顯現高訊號之特性而將血管圖像顯示出來的一種檢查。由於此檢查通常不需要注射對比劑,所以不具侵犯性。可利用此快速安全的方法來篩檢一些血管病灶,特別是顱內血管病灶,頸動脈狹窄及阻塞,或主動脈及其主要分枝血管病變等,我們在過去半年中有42例磁振血管攝影檢查,其中包括30例顱內磁振血管攝影,4例頸動脈磁振血管攝影,8例主動脈磁振血管攝影。我們發現磁振血管檢查對於顱內血管病灶如動靜脈畸形,動脈瘤極具診斷價值,可做為一種篩檢方法。對於頸動脈阻塞病灶可評估其阻塞位置及其側枝循環。對於主動脈及其主要分枝病變如主動脈剝離及腎動脈阻塞等亦可清楚顯現,以做術前評估及術後追蹤的參考。本文亦將探討磁振血管攝影的原理及其臨床應用。

Magnetic Resonance Angiography (MRA) gains an increasing importance as a screening examination for diagnosis of certain vascular lesions such as intracranial AVM and aneurysm. It is based on the particular feature of flowing blood under gradient echo sequence with flow compensation causing high signal of the flowing blood. There were 42 cases of MR angiography performed in our institution in 6 months, including 30 cases of intracranial MRA, 4 cases of extracranial carotid MRA and 8 cases of aortic MRA. We found that MRA is non-invasive and useful for diagnosis of intracranial vascular lesions, stenosis or occlusion of carotid artery. It is also useful to detect vascular lesion of aorta and its major branches such as aortic dissection and renal artery stenosis.
