Aortic rupture is a lethal injury readily treatable with prompt diagnosis in those patients who do not succumb at the scene. We report a rare case of complete aortic transection. A 21-year-old male was admitted for compressive chest pain and unstable blood pressure due to traffic accident. The clinical symptom and computed tomography demonstrated a rare but typical sign of ”acute traumatic aortic coarctation” phenomenon. The other CT findings of ruptured aorta in our case included (1) false aneurysm, (2) marginal irregularity of the opacified aortic lumen, (3) periaortic hematoma and (4) extravasation of high density contrast media.
