A patient is described in whom linear or fusiform hyperintensities near the filum terminale were shown on T1-weighted magnetic resonance (MR) images. Fat-suppression technique suppressed these high signals comparable to those of the epidural fat. Filar lipomas were initially diagnosed, but a plain lumbar spine radiograph revealed subdural Myodil accounting for the MR signal change. It is believed, thus, that fat-suppression technique cannot differentiate a fat-containing tumor from Myodil. Correlation with findings by plain radiograph is suggested whenever spinal lipoma is suspected on the basis of MR.
