摘要

一般之下顎疼痛或磨牙症(bruxism)通常由顳顎關節功能障礙(temporo-mandibular joint dysfunction)所引起，臨床之診斷相當困難，且可以有症狀卻無關節疾病。X光素片檢查及電腦斷層掃描可以提供部份診斷，卻無法查知顳顎關節半月板(meniscus)的異常。磁振造影(magnetic resonance imaging)可判斷顳顎關節之半月板前脫位(anterior dislocation)，對半月板破裂的診斷仍有困難。因此，除非進行更具侵襲性之關節鏡檢查，顳顎關節攝影術(temporomandibular joint arthrography)仍是顳顎關節協調不良病症無可取代之篩檢及診斷的工具。在這裡，我們提出台北榮民總醫院六十二例顳顎關節攝影術之經驗，並進一步分析及討論。

Lower jaw pain or bruxism is generally caused by temporo-mandibullar (T-M) joint dysfunction. The clinical diagnosis is rather difficult, and sometimes there are symptoms without joint illness. Plain x-ray film and computed tomographic examination can provide partial diagnosis, but there is still difficulty in diagnosis of meniscus tear. Therefore, unless more invasive arthorgraphy is undertaken, temporomandibular joint arthrography remains an unreplaceable screening and diagnotic tool in T-M joint dysfunction. This article presents experience with 62 cases of T-M joint arthrography in Veteran General Hospital-Taipei, with advanced analysis and discussion.
