摘要

Morton氏神經瘤不是真正的神經瘤而是足蹠神經週圍的纖維病變，我們報告兩例經手術證實發生於第三趾間的Morton氏神經瘤病例。病人是33歲及65歲的男性，主訴是腳的局部疼痛，尤其是在行走後，起初是骨科醫師以臨床症狀懷疑是Morton氏神經瘤，且因症狀無法緩解而準備進行手術。術前的以T1為主和以T2為主磁振影像發現，在第三及第四蹠骨頭間有低訊號強度的結節，與該結節之病理切片所發現的神經及血管週邊的纖維病變相吻合。第二例於注射顯影劑後發現該結節呈現週邊性訊號增強。磁振影像提供手術前正確的診斷與評估。回顧文獻，在臺灣地區Morton氏神經瘤的影像診斷極少被正式報告。

Morton's neuromn is nctnally n perineurni fibrosis of the plnntar digital nerve instead of a true ”neurorna”. We report two proven cases of Morton's neuroma in the third inter-metatarsal space. The patients are male adults, 33 and 65 years of age respectively, complaining of localized pain especially after walking. Diagnosis of Morton's neuroma were initially made by a senior orthopedist. Magnetic resonance imaging before surgical intervention discloses characteristic findings of hypointeuse nodule between the third and the fourth metatarsal heads on both T1 and T2 weighted images, which are consistent with pathological findings of perineural and perivascular fibrosis. Gadolinium-enhanced T1 weighted images in the second patient shows peripheral enhancement of the lesion. To our knowledge, Morton's neuroma is rarely reported in Taiwan.
