Experience of Endovascular Treatment of Direct Carotid-Cavernous Fistulas with Detachable Balloons
使用可分離式囊球血管內治療內頸動脈－海綿竇間瘻管之經驗
摘要
過去兩年間在本科有十四例外傷性內治療頸動脈－海綿竇間瘻管使用乳膠分離式囊球進行血管內治療。能成功地栓塞該瘻管有十三例(93%)。同時能保存內頸動脈的有八例(57%)。栓塞後，有一例病人出現短暫性外展神經麻痺。有一例病人因二次頭部外傷囊球破裂。有一例出現偽性動脈瘤。囊球移位或脫離的有兩例。從以上結果，我們相信這種高成功率低致病率的工具，是目前治療該疾病的最佳方法。

Fourteen procedures of endovascular treatment of traumatic carotid cavernous fistulas (CCFs) with latex detachable balloons were performed at our department in the past two years. Successful occlusion of the fistulas could be achieved in thirteen procedures (93%). Preservation of the parent internal carotid artery was possible in eight procedures (57%). One patient developed transient abducens palsy. One balloon ruptured due to repeat head trauma. Pseudoaneurysm developed in one case. Migration or dislodgment of the balloons was found in two cases. On the basis of these results, we conclude that the low morbidity and high successful rate currently make this modality to be the treatment of choice.
