A Modified Pigtail Technique for Angiographic Evaluation of Bilateral Femoral Arteries

股動脈攝影-以Pigtail導管為基礎的變革

摘要


股動脈數位血管攝影已逐漸成為評估下腹腔主動脈及兩側股動脈的標準方法。在大部分的情形下,兩側股動脈可以同時良好顯影,然而有時會因為兩側股動脈血管差異性過大而導致無法同時兼顧兩側股動脈血管造影。對此,我們提出以pigtail導管為基礎的股動脈攝影方法:在第一次數位血管造影的重點乃是腹腔主動脈及pigtail導管穿剌處(puncture site)對側的股動脈,然後將pigtail導管回抽至導引管(introducer)內,再進行第二次puncture site同側專一選擇股動脈血管攝影。相較以重覆性下腹腔動脈攝影來分別獲取兩側股動脈血管資訊,我們發現上述以pigtail導管為基礎的血管攝影方法,能以較少的顯影劑使用量及較佳的影像品質來提供兩側股動脈差異過大的下肢週邊血管資訊。

Digital subtraction angiography (DSA) using a bolus chasing method has become the standard method to evaluate the lumber aorta and bilateral femoral arteries. In the majority of cases, both arteries can be well opacified at the same time. However, on some occasions, there is a marked difference in opacification of the bilateral femoral arteries and their tributaries. Therefore, we devised a method in which a low lumber aortogram was performed with a pigtail catheter in the aorta with a pigtail catheter in the aorta with bolus catheter in the aorta with bolus chasing on the femoral artery contralateral to the puncture site, Then the pigtail was withdrawn with the tip inside the introducer and another angiogram was performed on the femoral artery at the puncture site. We used significantly less contrast material and achieved far better opacification of the femoral artery compared to using a repeat bolus chase with the pigtail catheter tip in the aorta on the other femoral artery. Our experience revealed that modified pigtail catheterization is an effective method for bilateral femoral arteriograms in cases of marked differences in opacification of the bilateral femoral arteries and their tributaries.
