Primary Liver Leiomyosarcoma - Mimicking Liver abscess

原發性肝臟平滑肌肉瘤非常少見，因此只有一些零星的病例報告。常見的臨床表現為肝臟腫大或上腹部腫塊且常合併有腹痛或體重減輕，我們報告一位60歲男性病患，臨床表現為偶發性的輕微發燒及右上腹痛，雙相電腦斷層攝影檢查發現一巨大病灶在肝臟右葉，其顯影形式在動脈相及門脈相均只有病灶壁上及中央間隔有顯影。雖然在臨床表現及電腦斷層攝影發現上懷疑為肝臟膿瘍，經皮肝臟穿刺抽吸檢查卻較懷疑為肝臟腫瘤，最後病患接受手術切除肝臟右葉，病理診斷為原發性肝臟平滑肌肉瘤。

Primary hepatic leiomyosarcoma is very rare with only sporadic cases reported. The usual clinical presentation is hepatomegaly or epigastric mass, which is commonly associated with abdominal pain or body weight loss. We reported a 60-year-old man who presented with intermittent low grade fever and right upper quadrant pain. Dynamic CT scan of liver revealed a big mass in right lobe of liver, which showed wall and internal septa enhancement in arterial and portal venous phase images. Although the clinical presentation and CT features suggested a hepatic abscess, a percutaneous aspiration procedure was performed and in favor of tumor growth thereafter. Right lobectomy was then performed. The final pathological report was high grade primary hepatic leiomyosarcoma.
