背景：鼻成形術可分為封閉式及開放式鼻成形術兩種，除能改善鼻子功能外，並能增進鼻部美觀，在本國內多由整形外科醫師為之。本研究主要報告三軍總醫院耳鼻喉科實施開放式鼻成形術的經驗。此開放式徑路可由耳鼻喉科醫師常規實施而達良好的手術效果。 方法：以回溯性分析自2001年7月至2002年8月間，共計有62 名患者接受同一作者實施的開放式鼻成形術，探討患者的臨床症狀、致病原因、診斷方法，以及手術的適應症。 結果：62名病患於全身麻醉下接受開放式鼻成形術共計64次，包括2次修正性手術。男性58名，女性4名，年齡分佈自15至50歲，平均為24.8歲。最常見求診的原因為鼻塞(85%)，其次為改善顏面美觀(68%)。致病原因多屬陳舊性外傷造成外傷性鼻扭曲有46名(74%)，無顏面外傷病史者有16名。除2名因術後外觀效果不佳而接受第2次手術外，病患手術結果鼻塞症狀均獲改善，美觀改善滿意度高，無傷口感染及其他併發症產生。 結論：開放式鼻成形術較封閉式鼻成形術有較佳的手術視野，能有效解除鼻塞並增進鼻部美觀，手術效滿意度佳，是耳鼻喉醫師可以發展的手術。

BACKGROUND: The aim of rhinoplasty is to establish certain aesthetic results while preserving satisfactory nasal function. The surgery, which can be divided to closed or open rhinoplasties, is mostly done by plastic surgeons in our country. The purpose of this study is to present the experiences of open rhinoplasties in Department of Otolaryngology, Tri-Service General Hospital. The external approach could be used by ENT surgeons frequently and routinely to achieve good surgical results. METHODS: The clinical symptoms and signs, etiology, diagnostic methods, surgical indications of open rhinoplasties performed by the first author from July1, 2001 to August 31, 2002 were retrospectively reviewed. RESULTS: Sixty-two patients (58 males and 4 females) underwent 64 open rhinoplasties under general anesthesia, including 2 revision operations. Their ages ranged from 15 to 50 years with a mean age of 24.8 years. The most common symptoms at the time of presentation were nasal obstruction (85%) and cosmetic deformity (68%). The most etiologies were previous trauma (74%). Obvious relief of nasal obstruction and good cosmetic result were achieved; however, 2 patients who were unsatisfied with the cosmetic results underwent revision rhinoplasties. The postoperative conditions were uneventful, and there were no wound infections and other complications. CONCLUSIONS: Open rhinoplasties give better surgical exposed field than closed rhinoplasties, and are a good selection for correction of nasal obstruction and cosmetic problems. Because of the good surgical results achieved, ENT surgeons can perform this type of procedure.
