摘要

非膽脂瘤慢性中耳炎，因症狀明顯，不容易被忽略。病人就醫的處理流程如下:1)局部檢查:檢查儀器包括充氣式耳鏡(pneumatic otoscope)、手術用顯微鏡(operating microscope)和硬式耳內視鏡(rigidoto-endoscope, tele-otoscope)等。2)外耳道分泌液的細菌培養和抗生素敏感測試。3)純音聽力檢查用來測量聽力損失的種類和程度。4)術前耳咽管功能檢查以便更清楚病變的範圍和潛藏的併發症。6)局部治療、耳滴劑和口服抗生素。7)鼓膜成形術(myringoplasty)或鼓室成形術(tympanoplasty)，不但可以去除病灶，消除耳漏，修復鼓膜穿孔，還可使傳音性聽力損失獲得改善。8)術後回診和定期追蹤。

Diagnosis of chronic otitis media (COM)is not difficult. The management of COM includes: 1. Local examination with pneumatic otoscope, operating microscope, rigid oto-endoscope and tele-otoscope. 2. Culture and sensitivity test of discharge in external ear canal. 3. Pure tone audiometry to reveal the type and severity of hearing loss. 4. Preoperative evaluation of Eustachian tube (E-tube) function. 5. Mastoid X-film to show the pneumatization and high resolution computerized tomogram of temporal bone to explore the extent of lesion and bony defect. 6. Topical use of otics combined with ear toilet, or parenteral antibiotics. 7. Eventual myringoplasty or tympanoplasty. 8. Postoperative care and follow-up.
