台灣年輕成人正常耳之鼓室圖分析

Analysis of Tympanogram of Normal Ears in Young Taiwanese Adults

背景:本研究的目的在建立台灣年輕成人正常耳鼓室圖四項參數的常模值:峰補償靜止聲導納(peak compensated static admittance, Ytm)、鼓室圖峰壓(Tympanometric Peak Pressure, TPP)、等耳道容積(equivalent ear canal volume, Vea)、以及鼓室圖寬度(tympanometric width , TW),並檢驗上述數值是否會因為男女性別或左右兩耳間有所差異,同時與其他國家之鼓室圖數據進行比較。方法:對象為聽力正常且無耳科疾病之台灣年輕成人300名(男140名,女160名),平均年齡20.30(+/- 3.15)歲。使用GSI 38中耳功能分析儀進行鼓室圖測量,記錄受測者兩耳的Ytm、T PP、Vea、TW等四項數值。結果:本研究所測得台灣年輕成人鼓室圖4項量化指標之平均值與標準差為: Ytm為0.5 mmho(+/- 0.2mmho),TPP為-9.8 daPa(+/- 15.0 daPa), Vea為1.1 cm^3 (+/- 0.3 cm^3), TW為89 daPa(+/- 28 daPa)。左、右耳在上述各項鼓室圖數值間無顯著差異;在男女性別方面,除了男性的Vea顯著大於女性外,其餘3項鼓室圖參數之平均值無顯著差異。結論:本研究結果提供了台灣年輕成人正常耳之鼓室圖量化數據,與國外研究比較後發現主要的差異在於:台灣年輕成人的Ytm平均值和90%正常範圍之上、下限值均明顯低於西方人,但與其它華人數據相近;TW較西方人寬;TPP偏負壓但皆在±50 daPa範圍內,與其他華人結果相近。建議鼓室圖判讀或轉介時,宜參照本土常模數據為宜。(台耳醫誌 2012; 47:24-31)

BACKGROUND: Tympanometry is a routine clinical measurement for testing the function of the middle ear. Several systems for classifying 226-Hz tympanogrms have been proposed and been widely utilized, however, four tympanometric parameters (peak compensated static admittance, Ytm; tympanometric peak pressure, TPP; equivalent canal volume, Vea; and tympanometric width, TW) can provide quantitative information in clinical diagnosis, Previous studies have noticed the importance of using a population-specific criterion for tympanometry testing. Thus, the objectives of the present study are: (1) to obtain tympanometric normative values in young Taiwanese adults; (2) to examine the gender and ear effects on tympanometric parameters; (3) to compare the results with data obtained from other studies for Chinese and White adults in Western studies.METHODS: 300 young adults with normal hearing were recruited in this study, using GSI 38 Auto Tympanometer.RESULTS: The normative data have been established for peak Ytm, Vea, TW, and TPP. No significant difference existed between left and right ear on values of the above four parameters, as well as those between male and female subjects, except the Vea on males' ears were bigger than the females.CONCLUSION: The tympanometric normative values of young Taiwanese adults has been established in this study. No apparent gender and ear difference existed this study except higher Vea values on males than those of females. No apparent difference was found on tympanometric normative data compared to those of Chinese population. (J Taiwan Otolaryngol Head Neck Surg 2012; 47:24-31)
