新制聽力障礙鑑定方式對於病患的影響

摘要

背景：自2012年7月11日起，新制的聽力障礙鑑定改為以雙耳的整體障礙比率取代過去的優耳聽力閾值來決定，本篇文章中分析了新制對於聽障病人造成的影響。方法：收集恩主公醫院2010年1月至2015年10月接受純音聽力檢查的病患(排除新制和舊制鑑定方式都不符合聽障資格者)，分析其純音聽力檢查報告，同一病患在相距半年內的報告僅採殘障等級較高者，比較用新制和舊制的鑑定方式在殘障鑑定的等級是否有明顯差異。結果：在4年又10個月期間符合分析標準之病患共356名，其中包含465份聽力報告，39份(8.38%)使用新制鑑定方式較舊制殘障等級為高，49份(10.53%)使用舊制鑑定方式較新制殘障等級為高。結論：在本次的分析中發現，約有兩成的檢查會因新制鑑定方式改變其殘障等級，八成左右的檢查並沒有影響，特別值得討論的是有3.62%的案例因為採行新制方得以通過鑑定，卻有8.52%的案例原先通過舊制鑑定者，在新制的標準下會無法通過。

BACKGROUND: Ministry of Health and Welfare in Taiwan had changed the formula for hearing disability certification since July 11, 2012. In the past, the hearing disability was graded by the average of hearing threshold at 500, 1000, 2000 and 4000 Hz of the better ear while the new grading system depends on the hearing handicap percentage of both ears. This article is aimed to investigate the impact of the new method on hearing disability certification. METHODS: We collect all the pure tone audiometry data in En-Chu-Kong Hospital since Jan 1, 2000 to October 31, 2015. We obviate those unqualified for hearing disability certificate neither by the past formula or the new one. If one patient had several audiometry report, we exclude those performed within 6 months and take the one which gets highest disability grade. RESULTS: 356 patients were included in our data with 465 pure tone audiometry reports. 39 audiometry data (8.38%) get higher disability grades with new method while 49 (10.53%) get lower grades. 40 audiometry reports (8.6%) which pass disability certification with the old formula now does not match the new certificate criteria while 17 (3.65%) which do not pass the past formula are now certificated for hearing disability with the new method. 377 (81.07%) audiometry reports have same disability grades no matter with the old or new formula. CONCLUSIONS: The formula for hearing disability certification was widely disputed in the past decade worldwide. According to our data, more patients are not qualified for disability certificate with the ongoing formula compared with previous one while a small percentage of patients get benefit from it, though.
