新型超音波裝置與睡眠多項呼吸生理檢查儀同步監測自然睡眠時舌根型態變化

A Novel Ultrasound Device for Tongue Base Contour Monitoring Synchronizing with Polysomnography during Natural Sleep

摘要


背景:睡眠多項呼吸生理檢查(polysomnography, PSG)是診斷阻塞性睡眠呼吸中止(obstructive sleepapnea, OSA)的標準方法,但無法提供患者上呼吸道阻塞部位及塌陷程度等相關資訊。本描述性研究探討以自行研發之可穿戴式經皮超音波裝置結合PSG同步監測自然睡眠時舌根型態變化的可行性。方法:9名健康志願受測者,5名女性4名男性,於接受PSG檢查時,配戴經皮超音波裝置同步進行記錄,此超音波裝置每秒自動記錄一次頸部皮膚表面至舌部空氣黏膜介面(air-mucosa interface)的最長距離(tongue base thickness, TBT)。受測者檢查完成後立即依睡眠時配戴此裝置的感受填寫問卷,並依據PSG所記錄的自然睡眠期間,分別計算男、女受測者TBT的平均值與標準差。結果:受測者平均年齡31.6 ± 9.2歲,平均身高163.9 ± 9.2 cm,平均體重61.4 ± 9.2 kg,平均BMI為22.8 ± 2.6 kg/m^2,平均apnea-hypopnea index (AHI)為0.7 ± 1.2,睡眠期間每名受測者之超音波平均記錄次數為7867 ± 2899次,所測得之TBT平均值為62.1 ± 0.8 mm (58.7-65.0),TBT最小值為44.0 ± 1.9 mm(37-54),TBT最大值為76.8 ± 0.8 mm (75-78)。配戴後之問卷顯示無皮膚過敏反應,無發熱感,配戴時整體不舒適感為輕微至中等程度。結論:本穿戴式經皮超音波裝置可結合PSG,即時且長時間同步記錄自然睡眠時舌根型態的變化,配戴時無不良反應,未來可作為判斷睡眠呼吸障礙(sleep-disordered breathing, SDB)患者舌根後區呼吸道塌陷程度之輔助工具。

BACKGROUND: Although polysomnography (PSG) is the gold standard for diagnosing obstructive sleep apnea (OSA), it cannot localize the sites of upper airway obstruction, and their degree of narrowing. This descriptive study aimed to explore the feasibility of a novel wearable transcutaneous ultrasound device in tongue contour monitoring, synchronizing with the PSG, during natural sleep. METHODS: Nine healthy participants (five women and four men) received synchronized PSG and ultrasound examinations. The maximum distance between the submental skin and the air-mucosa interface of the dorsal surface of the tongue base (tongue base thickness, TBT) was recorded by the ultrasound system automatically every one second. The subjects completed a survey regarding the comfortability of the wearable ultrasound device immediately after the examination. Furthermore, we calculated and compared the mean and standard deviation of the TBT, during natural sleep, for female and male subjects, respectively. RESULTS: The average age of the subjects was 31.6 ± 9.2 years; average height was 163.9 ± 9.2 cm; average weight was 61.4 ± 9.2 kg, and average BMI was 22.8 ± 2.6 kg/m^2. The mean apnea-hypopnea index (AHI) was 0.7 ± 1.2. The average ultrasound recording cycles during natural sleep were 7867 ± 2899. The average TBT was 62.1 ± 0.8 mm (58.7-65.0), while the minimal and maximal TBT were 44.0 ± 1.9 mm (37-54) and 76.8 ± 0.8 mm (75-78), respectively. A survey of the participants after the test showed no allergic reaction and no heat sensation over the submental skin. Uncomfortableness while wearing the ultrasound device ranked in the slight to medium degrees. CONCLUSIONS: The wearable transcutaneous ultrasound device, synchronizing with PSG, was capable of real-time and long-term recording of the TBT during natural sleep. The subjects reported no adverse reaction while wearing the device. In the future, the device can be used as a tool to assess the collapsibility of the retroglossal airway in sleep-disordered breathing patients.
