單側孤立性蝶竇疾患併發對側外弦神經麻痺－病例報告
Contralateral Abducens Palsy Secondary to Unilateral Isolated Sphenoid Sinus Disease-Case Report
摘要
本文作者報告一個案，為一45歲台灣女性，其臨床表現為長達1個月斷續且無法定位之深部頭痛並於就診前兩天出現複視現象，經理學及影像診斷為單側蝶竇疾患併發對側外弦神經麻痺。住院期間投予廣效型抗生素以及症狀控制藥物但症狀仍持續進展。經與病患充分討論後施予鼻竇內視鏡手術，病患的眼外肌運動得到完全的復原。據我們了解，本案例為英文文獻中，亞洲地區的第1例單側蝶竇疾患併發對側外弦神經麻痺。

We describe a case of contralateral abducens palsy secondary to unilateral isolated sphenoid sinus disease in a 45-year-old Taiwanese woman. Patient presented with intermittent but not localized deep headache for one month and diplopia for 2 days. After admission, broad spectrum antibiotics and analgesic medication were given but symptoms still progressing. After thoroughly discussed with patient, she was successfully treated with functional sinus surgery and the visual complication fully recovered. To the best of our knowledge, this is the first report of contralateral abducens palsy secondary to unilateral isolated sphenoid sinus disease in Asia.
