舌部異物肉芽腫臨床表徵與舌腫瘤相似－病例報告

摘要

因穿刺傷造成的體內異物，會使得組織內產生異物反應，局部發炎甚至是形成異物肉芽腫。本篇個案報告的病患是1名38歲女性病患，來本院門診的主述是過去的10個月間左側舌頭反覆腫脹並疼痛。在每次發作之後均使用抗生素治療，症狀都會在1至2週內消失。在門診理學檢查時發現左側舌部黏膜完整伴隨有一塊0.5 × 0.5 cm的區塊味蕾消失，觸診發現黏膜下可觸摸到約1 × 1 cm腫塊。門診中安排頭頸部電腦斷層檢查，報告顯示為左側舌部腫瘤，高度懷疑惡性的可能。病患於是進行腫瘤切片手術並同時冷凍切片病理化驗，結果顯示為急性合併慢性發炎組織，無惡性細胞。手術中將腫瘤完全切除，在切除的過程中發現檢體內包含尖銳的異物。腫瘤最後病理診斷為異物導致之舌部肉芽腫。

The foreign body in the human body from penetrated injury may cause a foreign body reaction which including local inflammatory response or formation of foreign body granuloma. We present a case of 38 years old female patient who came to our outpatient department due to left tongue intermittent submucosal swelling and tenderness for the past 10 months. Each swelling episode persisted for one to two weeks, and usually subsided after taking oral antibiotics. Physical examination showed isolated left dorsal tongue palpable bulging mass in submucosal layer. The tongue mucosa surface showed smooth texture with 0.5 × 0.5 cm area loss of papilla. Head and neck CT was arranged, and report suggested left tongue tumor, with highly suspected of malignancy. Therefore, we arrange incisional biopsy with specimen sent for frozen examination. The result showed acute and chronic inflammation, then total excision of left tongue tumor was done. An unexpected foreign body was found inside the specimen. The final impression of the tumor is foreign body in tongue muscle induced granuloma.
