Impending Extubation Failure after Transoral Robotic Surgery for Obstructive Sleep Apnea-Case Report

摘要


Transoral robotic surgery (TORS) is an alternative treatment for obstructive sleep apnea (OSA) patients who cannot tolerate continuous positive airway pressure well. We reported an OSA case with impending extubation failure event after TORS. We found that an omega-shaped epiglottis considerably contributed to this event, which was not highlighted in previous studies.

1名42歲男性患者,主訴打鼾多年。術前檢查顯示第3級扁桃腺肥大、舌根淋巴濾泡增生與omega型會厭。睡眠檢查顯示重度混合型睡眠呼吸中止症。於2019年10月28日接受達文西口咽舌根手術,術後帶著氣管內管回病房照護。術後第1天因狀況穩定拔管,但拔管後最低血氧濃度瞬間降低至62%,纖維內視鏡顯示omega型會厭被周圍腫脹軟組織嚴重擠壓,聲門呼吸道十分狹窄,緊急給予病人氧氣(流速6 L/min)與維持90度坐臥姿勢,逐漸改善,隔天完全不需給予氧氣。術後3個月追蹤發現舌根傷口和會厭軟骨有沾黏。先天omega型會厭者,合併舌根淋巴濾泡增生,若接受本手術,需注意omega型會厭在周圍腫脹組織壓擠下,有較高風險發生拔管後氣道阻塞問題。因不常見,特提出報告。
