帕金森氏病發生雙側聲帶麻痹－聲帶縫線外移手術成功脫離氣切管－病例報告

雙側聲帶麻痹為一複雜且可能影響聲音及呼吸方面功能異常之問題，甚至可能導致緊急的上呼吸道阻塞。造成此問題之原因相當多，而其中因帕金森氏病這種退化性疾患所產生之雙側聲帶麻痹則相對罕見。這類中樞性疾病若出現急性或慢性之上呼吸道阻塞問題常伴隨嗆食的問題，使治療更為複雜。在此我們提出一案例報告，單純以縫線將聲帶外展的手術方式，幫助1名因帕金森氏病造成雙側聲帶麻痹而長年須使用氣切之患者成功脫離氣切管之個案報告。

Bilateral vocal cord immobility can be a threatening condition affecting the function of phonation and respiration. The causes can be a wide variety including neurogenic and mechanic ones. The Parkinson's disease, as a degenerative brain disease and neurogenic one, rarely produces dysfunction of aerodigestive tract. The management of airway obstruction is usually complicated by the concomitant swallowing disorder. It may be mandatory to have a surgical intervention such as tracheostomy for an acute or chronic upper airway obstruction. However, the tracheostomy may severely affect the patient's quality of life. We will present a case report to introduce a simple surgical technique that lateralizes the vocal cord by a suture traction and successfully helps a patient with Parkinson's disease wean her tracheostomy.
