43年來翼狀片手術之變遷及臨床觀察

Clinical Observation and Review of the Pterygium Operation during 43 Year Period

Two hundred and seventeen eight cases of pterygium or 313 pterygium pieces were operated upon by the following techniques and the success rate was compared. These cases were collected from 1945 to 1969 and followed up for from 4 months to 19 years long. 143 cases were male, female 135 cases. Five methods used for operation were 1) Technique of Desmarres or Knapp and of McRaynolds 43 pieces 2) Technique of Spaeth 9 pieces 3) Technique of oral mucous membrane grafting (Duverger's method) 11 pieces 4) Technique of conjunctival grafting (of Komoto, or of Fox) 64 pieces 5) Baresclera method (Kunitomo-Mori) with Mitomycin instillation postoperatively 186 pieces Recurrence rate is the lowest, 5.49%, in Baresclera method, in conjunctival grafting method 9.37%, and much higher in the remaining methods. Technique of conjunctival grafting is easy and less in complication, so recommended strongly by the author. Though recurrence rate is low in bare sclera method with postoperative Mitomycin cinstillation, postoperative complication such as photophobia, foreign body sensation and glaucomatous symptom occur frequently. All advantage and disadvantage concerning these two methods were compared. Though the recurrence rate is slightly higher in the method of conjunctival grafting, the author prefers this method than bare sclera method because of less complication and good postoperative cosmetic apparance. So the method of conjunctival grafting is preferred by the author. Importance in eliminating the side effect of local Mitomycin after bare sclera method is stressed.
