十五年來臺大眼科眼球摘出之原因的研究

A Study on the Causes of Enucleation

This is a statistical observation on 382 cases of surgical enucleation performed at the Ophthalmological Department of the National Taiwan University Hospital over a period of fifteen years from 1957 to 1971. An effort was made to discover the primary cause, i.e. the condition leading to removal of the eye. Classification was based on etiologic as well as pathologic considerations. The frequency of causes leading to enucleation was injury (115 cases, 30.10 per cent), neoplasms (86 cases, 22.77 pet cent), absolute glaucoma (67 cases, 17.53 per cent), 1nfection and inflammations (51 cases, 13.35 per cent), phthisis bulbi (24 cases, 6.28 per ecnt), anterior staphyloma (16 cases, 4.18 per ecnt), and others (23 cases, 6.02 per cent). Injury was the most frequent cause for enucleation except the young age (under 10 years of age) and the old age (over 51 years of age). The male-female ratio was 7.21:1 (As contrast to the whole series, the male-female ratio was only 2.47 to 1). Post-traumatic complications especially infection and inflammtions were also important causes leading to enucleation. Almost all of the tumors leading to enucleation were retinoblestoma in infants and children (77 cases in 86 of total tumor cases), but there were 5 cases of malignant melanoma and 3 cases of metastatic carcinoma in adult and old age. 49 out of 86 tumor cases were males and the other 37 were females, the incidence appearing to be slightly higher in males, but it was the least ratio between males and females (1.32:1) in this series. Twelve eyes of the patients were enucleated due to complications of intraocular surgery. The types of operation performed prior to enucleation were cataract extraction (4 cases), glaucoma surgery (7 cases), and retinal surgery (1 case). The majority of complications leading to enucleation were panophthalmitis (3 cases), absolute glaucoma (8 cases), and anterior staphyloma (1 case).
