摘要

視乳頭周圍葡萄腫併發漿液性視網膜剝離，文獻上只有一篇報告提及，但却未加以治療。本篇報告這種罕見的疾病經由五次雷射光凝固治療的結果。病人為一38歲男性主訴兩個多月自覺性視野缺損。經檢查，視力右眼6/7.5，左眼6/6。左眼眼底正常，右眼眼底可見邊緣不明顯的視乳頭，位於一巨大深陷凹洞的顯側。自10:00至7:30方位均有視網膜剝離，上方及鼻側剝離的程度較輕，而下方的剝離甚為明顯，網膜無裂孔。偏顳側有明顯的視野缺損。入院後經氬氣雷射光凝固治療，施於剝離及剝離邊緣正常的視網膜上，其後門診追踪檢查一年，視力進步到6/6，視野擴大，眼底則見剝離的視網膜完全復位。本文就其併發漿液性視網膜剝離之發生機轉及其治療原則予以分別討論。

Peripapillary staphyloma is a rare congenital anomaly and is even rare when it associated with serous retinal detachment. We report such a case who was successfully treated by Argon laser photocoagulation. The patient was a 38 year-old otherwise healthy male. There's no visual symptoms until the patient found defective visual field of right eye 2 months before his admission. On admission, vision in the right eye was 6/7.5 and left eye 6/6. Visual field revealed marked depression of right temporal field. The eye conditions were entirely normal. In the right fundus, the normal optic disc was situated in a deep excavation. This cuplike excavation was 2 disc diameters in size and 10 diopters in depth, the choroid around the lesion showed mottled atrophic change. There's also serous detachment of retina from 10:00 to 7:30 o'clock position with macular spared. No retinal tear or tractional vitreous strand has been found. We performed the photocoagulation to wall off the detached retina and also covered the shallow portion of the detachment. After 5 times of photocoagulation, the retina was reattached and visual field extended and remained so after 1 year period of follow up. We also discuss the probable mechanism of serous retinal detachment which associated with peripapillary staphyloma.
