Juxtapapillary Retinal Angioma-A Case Report
視乳頭緣網膜血管瘤
摘要
本文報告一單側視乳頭緣網膜血管瘤之病例。病患為二十一歲女性，主訴為三個月之久的左眼視力模糊。左眼眼底檢查可見一位於視乳頭顳側，大小為1.5倍視神經盤直徑之橘紅色腫瘤；另有一環狀黃色之網膜下滲出物位於黃斑部之顳側。她亦無特殊異常之過去病史及家族史。 螢光血管攝影發現此腫瘤在動脈相早期即有螢光劑之充盈，灌注之動脈及引流之靜脈均清晰可見，晚期則有螢光劑之滲漏。 左眼視野檢查呈現與網膜腫瘤位置相對應之視野缺損。超音波檢查顯示一稍隆起腫瘤於視乳頭緣。左眼的視覺誘發電位圖則有明顯的減弱現象。理學檢查及腦部電腦斷層掃描均無特殊發現。 在十個月之觀察期間，此血管瘤無增大現象，且網膜下液亦逐漸消失，故對此病患未予治療。

A case of unilateral juxtapapillary retinal angioma in a 21-year-old female is reported. Fundus examination of the left eye revealed an orange-red round and well-defined tumor located at the temporal portion of the disc. Fluorescein angiography disclosed a sessile vascular lesion that filled with the retinal arterial circulation and leaked fluorescein into the subretinal space. The feeding and draining vessels were also clearly identified. Ultrasonography showed an elevated lesion on and around the region of the optic nervehead. ERG was normal in each eye. VEP was abnormal in the left eye. Neurologic evaluation failed to reveal any systemic evidence of von Hippel-Lindau disease. Differential diagnosis and management were discussed.
