正常眼、控制良好之青光眼及眼壓來控制之青光眼其坐臥姿眼壓之變化

Variation of Intraocular Pressure between Erect and Supine Positions in Normal Eyes, Controlled and Uncontrolled Glaucomatous Eyes

摘要


從七十年七月到七十一年四月以Haag-Streit裂隙燈上的Goldmann applanation眼壓計及手持式Perkins applanation眼壓計,分別量取115隻正常眼,106隻經手術或藥物治療後,眼壓控制良好之青光眼及38隻未經治療或治療失敗眼壓仍高之青光眼之坐臥姿眼壓。結果顯示,正常眼、控制良好之青光眼,及眼壓仍高之青光眼,其臥姿平均眼壓分別以3.8,1.8及5.6mmHg高於坐姿之平均眼壓。三者坐臥姿之眼壓差在統計學上有意義。坐臥姿眼壓改變之機轉,是因上鞏膜靜脈壓、脈絡膜血容積及血壓等改變綜合造成的。坐臥姿眼壓之差異可輔助青光眼之診斷及評估青光眼之治療成效;控制良好之青光眼,其坐臥姿眼壓差比平常眼為小;對於坐臥姿眼壓差大於正常者,需考慮有無青光眼,並應做更進一步的檢查。

The influence of postural change on intraocular pressure in normal eyes, controlled and uncontrolled glaucomatous eyes was studied. The intraocular pressures when the patients were in erect position were taken with Haag-Streit slitlamp mounted Goldmann applanation tonometer and those in supine position were taken with hand held Perkins applanation tonometer. The intraocular pressure was higher in supine position than that in erect position. The averages were 3.8, 1.8 and 5.6mm Hg respectively in normal eyes, controlled and uncontrolled glaucomatous eyes. Statistically significant differences were found between the normal and controlled glaucomatous eyes as well as between normal and uncontrolled glaucomatous eyes. Persons with higher than normal postural pressure change should be considered as possibly having glaucoma and undergo further provocative tests, while less than normal postural pressure change in those who had been treated for glaucoma might mean glaucoma was well controlled.
