摘要

視網膜表面的膜狀物，多半是一種增殖性反應的結果。如此一膜狀物發生於黃斑部或其附近，則可能引起視力障碍。在成功地摘除白內障或將剝離的視網膜復位之後，却發現黃斑表面膜狀物使患者視力逐漸減退，乃是一個令人苦惱的經驗。玻璃體切除手術發展之後，可藉手術顯微鏡及設計良好的器械，進行精巧的手術，將膜狀物剝離而完全除去，為本症的治療提供新的境界。作者報告在榮總應用Ocutome System，施行玻璃體手術，成功地去除兩例之黃斑表面膜狀物，而獲致視力的改善。

Epiretinal membrane formation is the result of a proliferative process. If this membrane formed near or on the macula, it may distort the macula or cover it, causing variable degree of visual disturbance. After an uneventful cataract extraction or reposition of the detached retina, it is an agonizing experience to find the patient's vision progressively get worse by the epimacular membrane. With the development of the vitrectomy, the opthalmic surgeon can perform a delicate operation to remove the epimacular membrane. The operation was aided by the operation microscope and precisely designed instrument. The vitrectomy procedure thus creates a new era in the management of the epimacular membrane. The author reports two cases of premacular fibrosis whose epimacular membrane were successfully peeled off with the Ocutome system and got visual improvement without any complication in two years.
